Todo bien // Hipertensión Ocular de Alan (Esp-Eng)

Hola, los saludos con mucho cariño desde la comunidad Motherhood. Ha pasado tiempo desde mi última publicación en este espacio especial para las mamás y también para los papás de Hive. La razón de mi publicación, es presentar una actualización sobre la salud visual de Alan luego de la última revisión con la Especialista en Glaucoma, por este año 2023. Ya algunos conocen el caso de mi hijo menor a quien se le diagnosticó hipertensión ocular y además hipermetropía, siendo atendido como un paciente con sospecha de glaucoma desde agosto del año 2022.

Para esta consulta médica tenía que llevar unos estudios solicitados, los cuales eran dos, una segunda tomografía u OCT de Discos y un Campo Visual. En todo salió bien Alan, gracias a Dios.

Actualmente Alan tiene 12 años y cuando tenía 11 empezó toda esta nueva historia médica de forma abrupta para él y para todos en la familia. En ese entonces estábamos en Venezuela pero a las pocas semanas viajamos a Colombia, donde es atendido regularmente por una doctora Oftalmólogo Pediatra y por otra doctora Oftalmólogo Especialista en Glaucoma. Cabe destacar que la atención que ha recibido Alan en Colombia ha sido excelente, se nota el profesionalismo muy bien combinado con humanidad, algo que todo paciente valora y más cuando es un niño con autismo.

Bien, como dije al principio Alan tuvo la última revisión de control, por este año, con la Especialista en Glaucoma y en cada revisión médica hay muchas revisiones a sus ojos. Como en toda consulta médica hay nervios, a pesar que Alan está muy acostumbrado a las revisiones y sabe el procedimiento que se debe seguir desde que llega a la clínica. Por ejemplo, al llegar y antes de pasar a la consulta, le hacen una primera medición de la PIO (Presión Intraocular).

Al pasar a la consulta se sienta en el sillón, como se lo indica la doctora para hacerle el examen visual. El debe ver letras que se proyectan desde un monitor que está en la pared del frente y las identifica bien.

El siguiente paso es observar el interior de ambos ojos y medir nuevamente la PIO (Presión Intraocular), la cual estaba en 18/18 MM HG, dentro de los límites normales en los niños. Ya antes he explicado que el mínimo debe ser 10 y máximo 20. A pesar que la tenía cerca del límite máximo, hay una ventaja que Alan tiene y es que su cornea es gruesa lo que hace la función como de colchón, algo así es lo que he entendido de los médicos y hay que restarle 6 a la PIO, entonces en realidad estaba en 12/12 MM HG. Para mí es un alivio porque Alan llegó a tener la PIO sobre 30/30 MM HG.

En la revisión física de sus ojos salió bien, igualmente los resultados de los estudios que le mandé hacer, la OCT de Discos y el Campo Visual. Lo que más me preocupa y por supuesto a los médicos es que su nervio óptico esté bien y la doctora encontró todo excelente en comparación con los estudios de hace un año, cuando se le diagnosticó la Hipertensión Ocular a Alan. Para la doctora, se nota el correcto cumplimiento del tratamiento con las gotas oftálmicas Latanoprost al 0.005%, que son las que usa cada 24 horas, todas las noches, para mantener controlada la hipertensión en sus ojitos.

Sin embargo, la doctora le indicó el uso de otras gotas oftálmicas para complementar el tratamiento, se llaman CONDROITIN Sulfato de Sodio, estas las debe usar cada 6 horas hasta nueva orden, para disminuir algunas incomodidades en los ojos como ardor, enrojecimiento, molestia por la luz y otras sensaciones que a Alan siente.

Gracias a Dios, todo va bien. Lo que debe estimularme a estar vigilante y ser responsable con todas las recomendaciones médicas, porque estar bien no debe ser motivo para descuidar el tratamiento, todo lo contrario porque mi hijo tiene antecedentes familiares de glaucoma.

The reason for my post, is to present an update on Alan's visual health after the last checkup with the Glaucoma Specialist, for this year 2023. Some of you already know the case of my youngest son who was diagnosed with ocular hypertension and also hypermetropia, being treated as a patient with suspected glaucoma since August 2022.

For this medical consultation he had to bring some requested studies, which were two, a second tomography or OCT of Disks and a Visual Field. Everything went well Alan, thank God.

Alan is currently 12 years old and when he was 11 years old this whole new medical story started abruptly for him and for everyone in the family. At that time we were in Venezuela but a few weeks later we traveled to Colombia, where he is regularly attended by a doctor Pediatric Ophthalmologist and another doctor Ophthalmologist Glaucoma Specialist. It is worth mentioning that the attention Alan has received in Colombia has been excellent, the professionalism is very well combined with humanity, something that every patient values and even more when it is a child with autism.

Well, as I said at the beginning, Alan had his last control check-up, for this year, with the Glaucoma Specialist and in every medical check-up there are many revisions to his eyes. As in every medical consultation there are nerves, even though Alan is very used to the check-ups and knows the procedure to follow since he arrives at the clinic. For example, when he arrives and before going to the consultation, he is given a first IOP (Intraocular Pressure) measurement.

When you go to the doctor's office, he sits in the chair, as the doctor instructs him to do the visual examination. He should see letters being projected from a monitor on the front wall and identifies them well.

The next step is to look inside both eyes and measure again the IOP (Intraocular Pressure), which was at 18/18 MM HG, within normal limits in children. I have already explained that the minimum should be 10 and maximum 20. Although it was close to the maximum limit, there is an advantage that Alan has and that is that his cornea is thick which makes the function as a mattress, something like that is what I have understood from the doctors and you have to subtract 6 from the IOP, so in reality it was in 12/12 MM HG. For me it is a relief because Alan got to have IOP over 30/30 MM HG.

The physical examination of his eyes came out well, as well as the results of the studies I had him do, the OCT of the discs and the visual field. What worries me most and of course the doctors is that his optic nerve is fine and the doctor found everything excellent in comparison with the studies of a year ago, when Alan was diagnosed with Ocular Hypertension. For the doctor, the correct compliance with the treatment with Latanoprost 0.005% ophthalmic drops, which are the ones he uses every night to keep the hypertension in his eyes under control, is noticeable.

However, the doctor indicated the use of other eye drops to complement the treatment, they are called CONDROITIN Sodium Sulfate, these should be used every 6 hours until further notice, to reduce some eye discomfort such as burning, redness, discomfort from light and other sensations that Alan feels.

Thank God, everything is going well. What should stimulate me to be vigilant and responsible with all medical recommendations, because being well should not be a reason to neglect the treatment, quite the contrary because my son has a family history of glaucoma.
