Hola, como ya saben estoy en el último año de mi carrera y desde que estudio prótesis estomatológicas muchos se preguntan que hago. Existen muchas ramas a estudiar, estas se dividen: según la cantidad de dientes a reponer puede ser una prótesis para desdentados totales o parciales, según el material con el que son confeccionadas pueden ser de acrílico o metálico, también pueden ser fijas o removibles como implantes o aparatos de ortodoncia, respectivamente. Sabiendo todas las especialidades procedo a darles un tour por mi laboratorio explicando más o menos algunos procesos de modo que entiendan o se lleven la idea de forma general.

Hello, as you know I am in the last year of my career and since I study stomatological prosthesis many people wonder what I do. There are many branches to study, these are divided: according to the amount of teeth to replace can be a prosthesis for total or partial edentulous, according to the material with which they are made can be acrylic or metal, can also be fixed or removable as implants or orthodontic appliances, respectively. Knowing all the specialties I proceed to give you a tour of my laboratory explaining more or less some processes so that you understand or get the idea in a general way.

El primer paso para hacer cualquier tipo de prótesis es la toma de impresión, este se hace en la clínica por el especialista y no es más que la copia de tus dientes. Esta impresión se rellena con yeso para obtener un modelo en yeso de tu boca. En dependencia de lo que quiera hacerse el paciente será la preparación del modelo, por ejemplo si quiere hacerse una prótesis total es necesario hacer una segunda impresión utilizando una cubeta individual que confeccionamos nosotros en el laboratorio, los otros tipos de restauraciones no requieren de este paso.

The first step to make any type of prosthesis is the impression taking, this is done in the clinic by the specialist and it is nothing more than the copy of your teeth. This impression is filled with plaster to obtain a plaster model of your mouth. Depending on what the patient wants to have done will be the preparation of the model, for example if you want to have a total prosthesis is necessary to make a second impression using an individual tray that we make in the laboratory, the other types of restorations do not require this step.

Utilizando un material termoplástico (significa que es un material que se ablanda con calor y se endurece cuando se enfría) creamos la placa base y encima hacemos un rodete de parafina con las medidas preestablecidas. Luego el paciente se prueba esta placa para que el especialista vea si tiene la altura morfológicamente correcta, si está muy alto se rebaja y si le falta altura se la repone. Con estas referencias estamos preparados para el siguiente paso, poner los dientes de acrílico y conformar la dentadura.

Using a thermoplastic material (which means that it is material that softens with heat and hardens when it cools down) we create the base plate and on top we make a kerosene impeller with the pre-established measures. Then the patient tries on this plate so that the specialist can see if it has the morphologically correct height, if it is too high it is lowered and if it lacks height it is replaced. With these references we are ready for the next step, put the acrylic teeth and form the denture.

Los dientes se eligen por tamaño y color, siguiendo las indicaciones del estomatólogo. Se colocan en la parafina siguiendo su orden y posición exacto para recrear una mordida perfecta. No es un trabajo fácil pues cualquier detalle puede afectar completamente al paciente, crearle lasceraciones en la boca o atrofiarle alguna articulación, pero se logra con mucha práctica. Cuando todos los dientes están bien colocados se le hace un encerado, que viene siendo como hacer una reproducción de la encía en cera y luego el paciente se lo prueba para comprobar que todo funciona como debe.

The teeth are chosen by size and color, following the indications of the stomatologist. They are placed in the kerosene following their exact order and position to recreate a perfect bite. It is not an easy job because any detail can completely affect the patient, create lacerations in the mouth or atrophy of any joint, but it is achieved with a lot of practice. When all the teeth are well positioned, a wax-up is done, which is like making a reproduction of the gum in wax and then the patient tries it on to check that everything works as it should.

Hecho esto, estamos listos para cambiar el material provisional por uno definitivo. En unas cajas metálicas llamadas muflas pondremos el modelo con la placa y los dientes, vamos a crear una camara de vacío. Para que me entiendan mejor, vamos a crear un molde en yeso, luego quitaremos los materiales primarios y rellenaremos ese molde con acrílico. Usamos acrílico rosado para simular el color de la encía, luego cerramos y lo prensamos, tiene que quedar bien prensado, que no entre aire. El acrílico que usamos es termopolimerizable, es decir, solo se endurece si le aplicamos calor entonces lo ponemos a cocinar en agua hirviendo o en microondas con unas muflas plásticas especiales.

Once this is done, we are ready to change the provisional material for a definitive one. In a metal boxes called muffles we will put the model with the plate and the teeth, we are going to create a vacuum chamber. So that you understand me better, we are going to create a mold in plaster, then we will remove the primary materials and we will fill that mold with acrylic. We use pink acrylic to simulate the color of the gum, then we close and press it, it has to be well pressed, so that no air gets in. The acrylic we use is thermo polymerizable, that is, it only hardens if we apply heat, so we put it to cook in boiling water or in the microwave with a special plastic muffle.

La prótesis está casi terminada, faltan los toques finales. Rebajamos con fresas y piedras las partes asperas o cortantes que puedan quedar en el acrílico con tal de eliminar las molestias al paciente. El último paso es el pulido, el toque final para darle una terminación bonita y lisa, lista para ir a la boca del paciente y devolverle la sonrisa.

The prosthesis is almost finished, the final touches are missing. We use milling cutters and stones to reduce the rough or sharp parts that may remain in the acrylic in order to eliminate any discomfort to the patient. The last step is the polishing, the final touch to give it a nice and smooth finish, ready to go into the patient's mouth and give him/her a smile back.

Pienso que no hay tarea más noble que devolverle a una persona sus dientes, tenemos la oportunidad de restaurarle las funciones como hablar bien o comer, que se vea bien y sobre todo que se sienta bien consigo mismo y eso para mí es hermoso. Es un proceso largo y un trabajo duro pero me encanta lo que hago, he aprendido mucho y me hace feliz compartirlo con ustedes. Espero que hayan aprendido algo y haya sido util, gracias por leer.

I think there is no nobler task than giving a person their teeth back, we have the opportunity to restore their functions such as speaking well or eating, that they look good and above all that they feel good about themselves and that for me is beautiful.
