Hace unas semanas comente que estaba por iniciar un nuevo semestre en la universidad y justamente eso me ha mantenido bastante alejada de Hive, sin embargo ese en concreto no es el tema del que quiero hablar hoy, resulta que la semana pasada comenzamos una serie de exposiciones donde hablaremos de diferentes discapacidades, así que me pareció interesante irles compartiendo semana a semana cada uno de los temas que se vayan tocando de forma tal que pueda irse convirtiendo en un conocimiento general que todos podamos poseer, justamente el primero de ellos es la discapacidad intelectual que muchos la conocemos como retardo mental, aunque este término realmente ya no se usa, asi que partiendo desde aquí comencemos con la publicación.

A few weeks ago I mentioned that I was about to start a new semester at the university and that is exactly what has kept me away from Hive, however that is not the topic I want to talk about today, it turns out that last week we started a series of presentations where we will talk about different disabilities, so I thought it would be interesting to share with you week by week each of the topics that will be discussed in such a way that it can become a general knowledge that we can all have, the first of them is the intellectual disability that many people know as mental retardation, although this term is not really used anymore, so starting from here let's start with the publication.

¿Qué es? || What is it?

Conocido mayormente como retardo mental, la discapacidad intelectual se considera un estado del funcionamiento cognitivo que resulta ser significativamente inferior al estado “normal” que debería presentar el paciente para su edad cronológica, comienza durante el curso de desarrollo y por tal motivo se considera un trastorno del neurodesarrollo, además de esto se caracteriza principalmente por las limitaciones del funcionamiento intelectual y deterioro de las capacidades adaptativas.

Known mostly as mental retardation, intellectual disability is considered a state of cognitive functioning that is significantly below the "normal" state that should be present for the patient's chronological age, begins during the course of development and is therefore considered a neurodevelopmental disorder, and is further characterised mainly by limitations of intellectual functioning and impairment of adaptive abilities.

Para el diagnostico de dicha discapacidad, la misma debe tener un inicio precoz, es decir antes de los 18 años, además deben presentarse deficiencias en las funciones intelectuales tales como el razonamiento, pensamiento abstracto, resolución de problemas, planificación juicio, aprendizaje académico entre otros, para ello se tienen que aplicar pruebas de inteligencia que se encuentren estandarizadas para la edad del paciente. De igual forma, deben encontrase alteraciones en las capacidades adaptativas del sujeto, las cuales pueden limitar el funcionamiento en las actividades de la vida cotidiana, si no son propiamente atendidas.

In order to diagnose this disability, it must have an early onset, i.e. before the age of 18, and there must also be deficiencies in intellectual functions such as reasoning, abstract thinking, problem solving, planning, judgement, academic learning, among others, for which intelligence tests that are standardised for the patient's age must be applied. Similarly, alterations must be found in the adaptive capacities of the subject, which may limit functioning in daily life activities, if they are not properly addressed.

¿Cuáles son los niveles? || What are the levels?

La discapacidad intelectual se presenta en 4 niveles de gravedad, en cada uno de ellos el funcionamiento adaptativo toma un rol fundamental, ya que es éste el que determina el nivel de apoyo que el paciente requiera. En este sentido, tenemos como primer nivel el leve (C.I: 69-50), este se caracteriza por un desarrollo tardío del lenguaje aun así los pacientes con este nivel son capaces de mantener una conversación y además la mayoría logra alcanzar una independencia en cuanto a su cuidado personal, las mayores dificultades se presentan a nivel escolar específicamente en la lectoescritura.

Intellectual disability is presented in 4 levels of severity, in each of them adaptive functioning plays a fundamental role, as it is this that determines the level of support that the patient requires. In this sense, we have as the first level the mild (I.Q.: 69-50), this is characterised by a late development of language even though patients with this level are able to maintain a conversation and also the majority manage to achieve independence in terms of their personal care, the greatest difficulties are presented at school level, specifically in reading and writing.

En el nivel moderado (C.I: 49-35) se presenta una lentitud en el desarrollo y comprensión del lenguaje, por lo que el nivel desarrollo de esta área puede variar desde adquirir la capacidad para lograr ser parte de una conversación sencilla, hasta solo lograr comunicar lo esencial para la vida cotidiana, el incluso existen pacientes que utilizan un lenguaje manual a manera de compensar la falta de comunicación verbal. Dentro del área escolar, solo llegan a aprender lo esencial para la lectura y escritura, además de esto presentan dificultades a la hora de su cuidado personal, por lo que suelen recurrir a supervisión permanente.

In the moderate level (I.Q: 49-35) there is a slowness in the development and understanding of language, so that the level of development in this area can vary from acquiring the ability to be part of a simple conversation, to only being able to communicate the essentials for daily life, and there are even patients who use manual language to compensate for the lack of verbal communication. In the school area, they only manage to learn the essentials for reading and writing, in addition to which they have difficulties with personal care, which is why they tend to resort to permanent supervision.

Dentro del nivel grave (C.I: 34-20), encontramos pacientes con cuadros clínicos más severos, aquí ya existe un escaso e incluso nulo desarrollo del nivel de lenguaje, además la mayoría suele presentar un marcado déficit motor, e incluso la presencia de otras carencias que pueden llegar a estar asociadas a un inadecuado desarrollo del Sistema Nervioso Central. Finalmente tenemos el nivel profundo (C.I: inferior a 20) los pacientes que se encuentran dentro de este grupo no poseen las capacidades para velar por su cuidado personal, por lo que requieren ayuda y supervisión constante, no logran atender instrucciones o reconocerlas para actuar conforme a ellas, su movilidad es bastante restringida o totalmente inexistente, de hecho suelen presentarse trastornos neurológicos graves que afectan la motricidad.

Within the severe level (I.Q: 34-20), we find patients with more severe clinical pictures, here there is little or even no development of the language level, in addition the majority usually present a marked motor deficit, and even the presence of other deficiencies that may be associated with an inadequate development of the Central Nervous System. Finally, we have the profound level (I.Q.: below 20), patients in this group do not have the capacity to look after their own personal care, so they require constant help and supervision, they are unable to follow instructions or recognise them in order to act in accordance with them, their mobility is quite restricted or totally non-existent, and in fact there are usually serious neurological disorders that affect their motor skills.

La discapacidad intelectual y el autismo suelen presentarse en conjunto, por lo que es bastante usual que se den diagnósticos errados e allí la importancia de hacer un seguimiento detallado de estos casos, buscando siempre la actualización de los diagnósticos, así como contrastar con diversos especialistas. El trabajo con especialistas (médicos, psicólogos, psicopedagogos) es importante para brindar una adecuada calidad de vida en los pacientes de discapacidad intelectual, pero si este trabajo no va acompañado de un interés genuino por parte de los familiares, es poco probable ver resultados exitosos.

Intellectual disability and autism usually occur together, so it is quite usual that misdiagnoses are given, and that is why it is important to make a detailed follow-up of these cases, always looking for the update of the diagnoses, as well as to contrast with different specialists. The work with specialists (doctors, psychologists, psychopedagogues) is important to provide an adequate quality of life in patients with intellectual disabilities, but if this work is not accompanied by a genuine interest on the part of the relatives, it is unlikely to see successful results.

There are many different origins in intellectual disability, autisms, chromosome trisomy, or even accidents with no genetic background. Difficult to add everything in the same pot. Thanks for the post

It is absolutely true, in fact the variability of the origin of this disability is one of the main factors that confuse its diagnosis, that is why it is necessary to be well informed.

Saludos, bienvenida a la comunidad y gracias por traer esta temática, de suma importancia no sólo por la parte informativa sino por el hecho de valorar a la persona en sí misma, más allá de cualquier tipo de discapacidad, el hecho de que le hayan cambiado el nombre de "retraso" es un avance, además de que también hay que considerar que actualmente la inteligencia no se mide solo a nivel de conocimiento sino que otros factores, otros tipos de entendimiento de la realidad, como la inteligencia emocional por ejemplo.
