Hidronefrosis// Hydronephrosis.

Hola amigos de Hive, un placer saludarlos y un verdadero honor estar acá nuevamente. No puedo dejar de anhelar para ustedes salud, vida y buena actitud, para que todas las metas que se propongan puedan ser logradas de manera exitosa.

En mi época de estudiante de medicina, tuve grandes maestros, algunos muy teóricos, otros más prácticos, la mayoría muy serios, otros impartían la cátedra con una gran sonrisa.

En el camino también, me encontré con profesores carismáticos y otros no tanto, de igual manera intenté aprender de sus conocimientos, sacar cosas positivas de su manera de interpretar la vida y la medicina, y escuchar cada consejo en pro de mi crecimiento personal y profesional.

Entre los recuerdos que atesoro, está una frase que quedó marcada en mi mente, fue en 5to año de mi carrera mientras hacía rotación hospitalizaría, mi profesor el Doctor Peter Figueira quien es Cirujano, Oncólogo y Ecografista, mientras realizaba un ultrasonido, me dijo "para ser un buen médico, debes ser una buena persona".

La frase que pronunciaron sus labios, no sé si le pertenecen, pero definitivamente marco mi vida, dicha frase la pronunció a las 3 de la mañana, mientas le realizaba el estudio a un paciente de bajos recursos, proveniente de un pueblito, quien estaba presentando una hidronefrosis grado II.

Luego de oír aquellas palabras, estudie mucho sobre esa patología, y es el tema que hoy les traigo Hidronefrosis.

La Hidronefrosis Es la inflamación debido a la acumulación de orina, y puede darse en uno o con menos frecuencia en ambos riñones, por tanto la vejiga no puede excretar adecuadamente el líquido acumulado.

Es importante mencionar, que la hidronefrosis no es una patología como tal, sino una señal, de que está ocurriendo alguna afección a nivel renal. Asimismo se debe mencionar que la hidronefrosis no respeta edad, sexo, ni condición y puede manifestarse en hombres, mujeres y niños.

Entre los factores frecuentes que pueden conducir a una Hidronefrosis se encuentran los siguientes:

1. Obstrucción de uréter producto de infecciones repetitivas, cirugías o radiación.

2. Obstrucción por cálculos renales.

3. Bloqueo de útero agrandado motivo de Embarazo.

4. Defectos congénitos del aparato urinario.

5. Formaciones de masas tumorales en vejiga o conductos urinarios.

Dentro de este orden ideas, se debe mencionar que las obstrucciones pueden hacer su aparición de forma repentina o se puede instalar gradualmente en cualquier caso debe ser tratada con rapidez pues este tipo de afección puede generar daño permanente a los riñones.

Actualmente en Venezuela se plantean 4 tipos de hidronefrosis y se identifican radiológicamente:

Grado I: Dónde la pelvis esta ligeramente dilatada, sin compromiso calicial.

Grado II: Acá la pelvis esta ligeramente dilatada y hay un leve inflamación calicial.

Grado III: Pelvis Dilatada, parenquima normal, hay una inflamación calicial marcada.

Grado IV: Pelvis muy dilatada, parenquima renal estrechó, cálices muy grandes.

Síntomas:

Dolor en un costado de la espalda, ansiedad, vomito, fiebre, deseos constantes de orinar, reducción en el flujo urinario.

Complicaciones:

Las complicaciones más frecuentes son infecciones renales (pielonefritis) e insuficiencia renal, y menos frecuentes hipertensión arterial.

Tratamiento:

El tratamiento irá en dependencia del grado de hidronefrosis que tenga el paciente, y la patología que está causando está enfermedad.

El médico tratante, puede indicar antibióticos, pasando por el uso de sondas vesicales, así como también si la obstrucción es severa, la resolución puede llegar a tener resolución quirúrgica. En todo caso se debe hacer énfasis que cada situación puede ser distinta.

Este tema es apasionante y muy extenso, en mi formación aprendí un poco sobre ella, y también aprendí que ser buena persona es indispensable para ser un buen médico, que no importa la posición social del paciente cuando la vocación existe.

Hydronephrosis is the inflammation due to the accumulation of urine, and it can occur in one or less frequently in both kidneys, therefore the bladder cannot excrete the accumulated liquid properly.

It is important to mention that hydronephrosis is not a pathology as such, but a sign that a kidney condition is occurring. It should also be mentioned that hydronephrosis does not respect age, sex or condition and can manifest itself in men, women and children.

Among the frequent factors that can lead to hydronephrosis are the following:

1. ureter obstruction resulting from repetitive infections, surgeries or radiation.

2. Obstruction due to kidney stones.

3. Blockage of enlarged uterus due to pregnancy.

4. Congenital defects of the urinary system.

5. Formation of tumor masses in the bladder or urinary ducts.

Within this order of ideas, it should be mentioned that the instructions can appear suddenly or can be installed gradually in any case should be treated quickly because this type of condition can cause permanent damage to the kidneys.

Currently in Venezuela there are 4 types of hydronephrosis and they are identified radiologically:

Grade I: Where the pelvis is slightly dilated, without calcific involvement.

Grade II: Here the pelvis is slightly dilated and there is a slight calcific inflammation.

Grade III: Dilated pelvis, normal parenchyma, there is a marked calcific inflammation.

Grade IV: Pelvis very dilated, renal parenchyma narrowed, very large calyces.

Symptoms:

Pain in one side of the back, anxiety, vomiting, fever, constant urge to urinate, reduced urinary flow.

Complications:

The most frequent complications are renal infections (pyelonephritis) and renal failure, less frequent arterial hypertension.

Treatment:

The treatment will depend on the degree of hydronephrosis that the patient has, and the pathology that is causing this disease.

The treating physician may prescribe antibiotics, including the use of bladder catheters, as well as if the obstruction is severe, the resolution may be surgical. In any case it should be emphasized that each situation may be different.

This topic is exciting and very extensive, in my training I learned a little about it, and I also learned that being a good person makes better doctors, that it does not matter the social position of the patient when the vocation exists.
