(ESP-ENG) OSTEOSARCOMA.

Hola amigos de Hive, un placer saludarlos nuevamente, como siempre mis deseos de salud óptima para ustedes, ya que sin duda la salud es nuestro mayor tesoro, si la tenemos con perseverancia podemos lograr todo lo que nos propongamos.

Les cuento que hoy fue día de revista general en el postgrado que estoy realizando (medicina interna). Es de costumbre que dichas revisitas se hagan a diario, a las 8 de la mañana, con la peculiaridad que los días martes, están presente todos los especialista que integran el hospital donde me desempeño.

Los especialistas antes mencionados, eligen un trío de residentes ( de tercero, de segundo y de primer año), para presentar el caso de mayor relevancia en los pacientes hospitalizados.

Pues, bien fui elegido para presentar el caso clínico el día de hoy, y gracias a Dios, y a mis otros compañeros tuvimos un desenvolvimiento notable.

Lo anteriormente expresado lo hago pues, el caso tratado fue de un paciente con osteosarcoma, tema que me parece oportuno compartir con ustedes.

El Osteosarcoma:

Es un tipo de cáncer óseo, que se presenta con mayor frecuencia en adolecentes y adultos jóvenes,sim embargo puede producirse en cualquier etapa de la vida.

Este tipo de neoplasias malignas, es la más común con entre los tumores que afectan a los huesos, está marcada principalmente por la formación de tejido óseo inmaduro con presencia de crecimiento anormal de células alrededor del hueso.

Este tipo de afección, tiende afectar mayormente a los huesos largos sobretodo a nivel de extremidades inferiores ( fémur, tibia).

Es importante mencionar, que los osteosarcomas no tienen una causa específica definida, sin embargo no se descarta la radiación ionizante en algunos casos, así como la predisposición genética, que sin duda tienen incidencia en algunos casos.

Síntomas:

Entre los síntomas más comunes de encuentra el dolor e inflamación a nivel óseo, sin haber sufrido ningún traumatismo, ni realizado ni realizado ninguna actividad que cause estrés sobre la zona afectada, además se asocia limitación a movimientos, enrojecimientos y fracturas con facilidad.

Diagnóstico:

Es importante el interrogatorio al pacientes, identificando sus antecedentes familiares (ya que el factor genético es importante, como se mencionó antes),así como también su medio social y laboral, para identificar posibles factores de riesgo.

Mientras que entre los exámenes para diagnosticar los osteosarcomas se encuentran: los rayos x, tomografía axial computarizada, gammagrafía ósea, resonancia magnética, y exámenes de sangre, entre los cuales se encuentra la hematología completa que con frecuencia, muestra una baja considerable de hemoglobina, y glóbulos blancos, y una aumentó de plaquetas (trombocitosis), asimismo se indican marcadores tumorales.

Tratamiento:

Los pacientes con este tipo de patología deben tener un equipó multidisciplinario a su disposición, entre los cuales destacan oncólogos, cirujanos oncólogos, psiquiatra, médico internista y enfermeros, quien sin duda aportarán su experiencia y conocimiento para que la afección sea tratada.

Entre el protocolo descrito está el tratamiento quirúrgico y las quimioterapias que cumplirán su función una vez determinado el grado en el cual se encuentre neoplasia en cuestión.

Actualmente está descrita tratamientos en fases experimentales como lo son bisfofonatos e inhibidores de proteínas cinasas, los cuales han demostrado eficacia en sus primeras fases, sin embargo es muy pronto para hablar de una terapia cien por ciento eficaz.

De igual manera, la complicación más temida de este tipo de tumor es la metástasis, mientras su pronóstico va estrechamente relacionado la detección temprana de la enfermedad y el grado en que se encuentre, aunque habitualmente es favorable.

Debo decir que cada uno de nosotros es responsable de nuestra propia salud, es por ello que recomiendo a mis pacientes visitas regulares al médico y exámenes periódicos, prevenir también es cuidarse.

Well, I was chosen to present the clinical case today, and thanks to God and my other colleagues we had a remarkable performance.

The above I do so because the case treated was a patient with osteosarcoma, a topic that I think it is appropriate to share with you.

Osteosarcoma

It is a type of bone cancer, which occurs more frequently in adolescents and young adults, however it can occur at any stage of life.

This type of malignant neoplasm is the most common among the tumors affecting the bones, it is mainly marked by the formation of immature bone tissue with the presence of abnormal cell growth around the bone.

This type of affection tends to affect mostly the long bones, especially at the level of the lower extremities (femur, tibia).

It is important to mention that osteosarcomas do not have a specific defined cause, however ionizing radiation is not ruled out in some cases, as well as genetic predisposition, which undoubtedly have an incidence in some cases.

Symptoms:

Among the most common symptoms of is pain and inflammation at bone level, without having suffered any trauma, or performed or performed any activity that causes stress on the affected area, also associated limitation to movements, redness and fractures easily.

Diagnosis:

It is important to question patients, identifying their family history (since the genetic factor is important, as mentioned above), as well as their social and work environment, to identify possible risk factors.

While among the tests to diagnose osteosarcomas are: x-rays, computed axial tomography, bone scintigraphy, magnetic resonance imaging, and blood tests, among which is the complete hematology that frequently shows a considerable drop in hemoglobin, and white blood cells, and an increase in platelets (thrombocytosis), tumor markers are also indicated.

Treatment:

Patients with this type of pathology should have a multidisciplinary team at their disposal, including oncologists, oncological surgeons, psychiatrists, internists and nurses, who will undoubtedly contribute their experience and knowledge so that the condition can be treated.

Among the protocol described is the surgical treatment and chemotherapies that will fulfill their function once the degree of neoplasia in question is determined.

Currently, treatments in experimental phases are described, such as bisphosphonates and protein kinase inhibitors, which have shown efficacy in their first phases, however, it is too early to speak of a one hundred percent effective therapy.

Similarly, the most feared complication of this type of tumor is metastasis, while its prognosis is closely related to the early detection of the disease and the degree in which it is found, although it is usually favorable.

I must say that each one of us is responsible for our own health, that is why I recommend my patients regular visits to the doctor and periodic examinations, prevention is also taking care of yourself.
