Una nuevo día nos regaló Dios Bendito, y debemos estar agradecidos por tan maravilloso obsequio y a pesar de las adversidades propias de la vida, acá seguimos.

Hoy escribiré un poco sobre Cirrosis Hepática.

La cirrosis hepática es una patología que afecta al hígado, es una enfermedad de carácter crónica, irreversible y progresiva y aunque normalmente las personas asocian la cirrosis con alcoholismo, esta tiene variabilidad de causas.

Debo destacar que esta enfermedad es más frecuente en hombres mayores de 20 años. Actualmente la Organización Mundial de la Salud (OMS), estima que se producen más de 800.000 muertes a causa de esta patología.

Esta afección tiene gran impacto en la calidad de vida de quien lo padece, generando en el individuo una insuficiencia hepática

Desde mi experiencia, puedo decir que la mayoría de las cirrosis no presentan síntomas en sus inicios. De hecho, muchas veces la enfermedad es diagnósticada de forma accidental. El paciente muchas veces acude a consulta por diarreas, o dolor en el epigastrio, y cuando se indican los exámenes completarios, se evidencian resultados compatibles con dicha patología.

Una vez avanzada la cirrosis, el paciente suele presentar:

Fiebre moderada y frecuente, dolor en hipocondrio derecho, flatulencias, síntomas neuropsíquicos que suelen cursar con episodios de somnolencia y pueden llevar al coma por encefalopatías de carácter hepático, coloración amarilla de piel y mucosas (ictericia), hemorragias espontáneas sobre todo a nivel de nariz y encías, arañas vasculares en la cara, cuello y miembros superiores, edemas en miembros inferiores y cuando la enfermedad está muy avanzada cursa con ascitis (acumulación de líquido en cavidad abdominal).

Etiología:

Como al principio comenté, Si bien es cierto que mayoría de las cirrocis hepáticas están asociadas al consumo de alcohol, pues ocupan al menos el 60% de las causas de esta patología generando una fibrosis en el hígado que a su vez produce una insuficiencia hepática, no es el único motivo de esta afección.

El virus de la Hepatitis B y C también son causales de esta enfermedad, que muchas veces no son diagnosticadas ni tratadas a tiempo lo que a la larga, generará una cirrosis.

Otras causas menos frecuentes es esteatohepatitis no alcohólica, la hepatitis autoinmune, colestasis obstructiva crónica, hemocromatosis hereditaria, enfermedad de Wilson entre otras.

Diagnóstico

El diagnóstico podemos llevarlo a cabo a través de un interrogatorio profundo donde se pongan de manifiestos los antecedentes epidemiológicos del paciente, su modo y estilo de vida, además de un examen físico exhaustivo, completando con los exámenes correspondientes entre los cuales se destacan, una prueba en sangre de bilirrubina total e indirecta.

De igual forma un ultrasonido hepático que nos daría una visión del órgano en cuestión, y una tomografía axial computarizada para saber el daño causado de la enfermedad.

Tratamiento:

El tratamiento suele variar dependiendo del agente causal de la cirrosis, sin embargo en todos los casos de indican dietas ricas en proteínas y fibra, bajo en grasas y calorías, para de esta forma no colapsar al órgano afectado (hígado) y que posee muchas funciones vitales para nuestro organismo.

El consumo de alcohol está totalmente prohibido, al igual que los medicamentos hepatóxicos entre los cuales encontramos la aspirina, paracetamol, ibuprofeno entre otros, asimismo el paciente debe mantenerse bien hidratado.

No podemos olvidar que la cirrosis es una enfermedad que requiere atención médica rápida, especializada, personal y oportuna para de esta forma tomar las mejores decisiones terapéuticas posibles.

Liver cirrhosis is a pathology that affects the liver, it is a chronic, irreversible and progressive disease and although people normally associate cirrhosis with alcoholism, it has variable causes.

I must emphasize that this disease is more frequent in men older than 20 years. Currently, the World Health Organization estimates that there are more than 800,000 deaths due to this pathology.

This condition has a great impact on the quality of life of those who suffer from it, generating liver failure in individuals,

From my experience I can say that most cirrhosis does not present symptoms in its beginnings, in fact many times the disease is diagnosed accidentally, the patient often comes to the clinic for diarrhea, or pain in the epigastrium, and when indicated The complementary exams show results compatible with said pathology.

Once cirrhosis is advanced, the patient usually presents:

Moderate and frequent fever, pain in the right hypochondrium, flatulence, neuropsychic symptoms that usually occur with episodes of drowsiness and can lead to coma due to hepatic encephalopathies, yellow coloration of the skin and mucous membranes (jaundice), spontaneous hemorrhages, especially in the nose and gums, spider veins on the face, neck and upper limbs, edema in the lower limbs and when the disease is very advanced it presents with ascites (accumulation of fluid in the abdominal cavity).

Etiology:

As I mentioned at the beginning, although it is true that most liver cirrhosis are associated with alcohol consumption, since they occupy at least 60% of the causes of this pathology, generating fibrosis in the liver that in turn produces liver failure, is not the only reason for this condition.

Hepatitis B and C viruses are also causes of this disease, many times these diseases are not diagnosed or treated in time, which in the long run will generate cirrhosis.

Other less frequent causes are non-alcoholic steatohepatitis, autoimmune hepatitis, chronic obstructive cholestasis, hereditary hemochromatosis, Wilson's disease, among others.

Diagnosis

We can carry out the diagnosis through an in-depth interrogation where the epidemiological background of the patient, his way of life and lifestyle are revealed, in addition to an exhaustive physical examination, completing with the corresponding examinations, among which stand out, a test in total and indirect bilirubin blood.

In the same way, a liver ultrasound that would give us a vision of the organ in question, and a computerized axial tomography to know the damage caused by the disease.

Treatment:

The treatment usually varies depending on the causal agent of cirrhosis, however in all cases diets rich in protein and fiber, low in fat and calories, are indicated, so as not to collapse the affected organ (liver) and that it has many functions vital for our body.

The consumption of alcohol is totally prohibited, as well as hepatoxic medications, among which we find aspirin, paracetamol, ibuprofen, among others, and the patient must stay well hydrated.

We cannot forget that cirrhosis is a disease that requires rapid, specialized, personal and timely medical attention in order to make the best possible therapeutic decisions.
